Abstract

Treatment of gingival recession has become an important therapeutic issue due to patients increasing aesthetic demand. Several surgical techniques have been proposed to obtain coverage of the exposed root surface in order to restore aesthetic. Two main group of therapy can be identified pedicle graft and free graft. The pedicle graft has been recently classified as rotational and advanced flap, for the rotational graft the only available root coverage data are those for sliding flap and reports of root coverage vary from 56% to 74% of previously exposed root surface, higher percentage of root coverage has been reported for the advanced flap 80% to 97% showing complete root coverage for the coronally advanced flap. Soft tissue taken from the palate described as epithelium-connective tissue and connective tissue graft, the surgical technique was modified over the time but clinical results remain controversial, the percentage of root coverage varies between 11% and 100%. According to Miller, achieving complete root coverage is possible in class I & II recession defects primarily because soft tissue and bone are still intact. Are this enough to have a predictable root coverage of previously denuded root surface and resulting in improving aesthetic? The presentation will give an overview of how current evidence based periodontics can predictably contribute to the management of gingival recession.
